General Requirements:

1. Original passport (6 months valid prior to departure date)
2. Copy of the bio-data page of the passport
3. Two (2) passport size photographs (white background)
4. A fully completed Application for Visa Form (DHA84) – MUST BE A4, BACK TO BACK (downloadable form visit www.southafricamanila.com)
5. Flight Itinerary/Airline Booking Proof of accommodation (hotel booking/Address of SA contact)
6. Proof of sufficient funds e.g. bank statement or certificate (original/certified copy) with latest 3 months bank statement
7. NSO Original Birth Certificate
8. NSO Marriage Certificate
9. Guarantee letter from the Travel agency listing of the passenger’s name & certifying that the said passengers availed package tour & will be back in Manila after the tour.

For Employee:

• Original Certificate of Employment

For Businessman:

• Photocopy of Business Documents

For Student/Child:

• Photocopy of School I.D.
• Original Affidavit of Support (Notarized)
