General Requirements:

1. Valid passport (6 months valid upon return)
2. 2 passport size color photograph
3. Completed application form
4. Employment Certificate and Approved Leave of Absence (for employees) with 6 months payslips
5. Business documents such as; Mayor’s Permit, DTI/SEC (for businessman)
6. Latest Income Tax Return
7. Bank Certificate, last 6 months bank statement and last 6 months credit card statement
8. Birth Certificate (authenticated by the NSO)
9. Marriage Certificate (authenticated by the NSO – for married)
10. Proof of assets such as; Land Title, Car Registration, or any property documents
    • All documents submitted should be original and with one (1) set of photocopy.
11. Previous passports (within 10 years)
12. Travel Health Insurance 30,000 Euro coverage
